Please find the attached Request for Proposal (RFP) 0001185 for Occupational Medicine Clinic Services. The purpose of this subcontract is to provide a turnkey Occupational Medical Services Clinic of superior quality in support of on-going work activities by CONTRACTOR and its employees. SUBCONTRACTOR shall maintain a “readiness to operate” capability sufficient to support the following activities for a workforce of approximately 700 employees.

Please note the following critical schedule related items: 

• Planned Response to the solicitation by sending an acknowledgement email to n3bprocurementopportunities@EM-LA.DOE.GOV by 7/12/2023.
• Subcontractor Questions are Due to N3B on July 24, 2023 at 12:00 PM MST and should be submitted to n3bprocurementopportunities@EM-LA.DOE.GOV.
• N3B Response to Questions will be published no later than July 31, 2023 COB and provided via email transmission.
• The proposals are due to N3B on Monday August, 21 2023 at 3:00 PM MST and shall be submitted to n3bprocurementopportunities@EM-LA.DOE.GOV.

Please find the RFP 0001185 Addendum #01 for Occupational Medicine Clinic Services.

Addendum #01 incorporates the following:

1. Extends the Proposal Due Date to:  3:00 PM MST, Monday, August 21, 2023
2. Extends the Questions Due Date to:  All questions related to the solicitation shall be submitted no later than 7/24/2023 12:00 PM MST
3. Extends the N3B Answers Date to:  N3B will post answers to the questions no later than 07/31/2023.
4. Replace RFP No. 0001185, N3B Form 9072, Rev. 1, with RFP No. 0001185 Addendum #01, N3B Form 9072, Rev. 2, dated 7/17/2023

No other changes have been made to the Solicitation.

Please find the RFP 0001185 Addendum #02 for Occupational Medicine Clinic Services.

Addendum #02 is issued to provide answers to questions submitted to N3B by the Offerors.

Additionally, Addendum #02 is issued to provide updates to N3B required Forms and Exhibit D Scope of Work and Technical Specifications.

1. Exhibit C Form E Key Personnel and Associated Resumes dated June 7, 2023 has been updated and changes are reflected in Exhibit C Form E Key Personnel and Associated Resumes, Rev.1 dated July 24, 2023.
2. N3B Form 9072, Request for Proposal (RFP)-0001185 Rev.2 dated 7/17/2023 has been updated and changes are reflected in N3B Form 9072, Request for Proposal (RFP)-0001185 Rev.3 dated 7/20/2023.
3. Exhibit D Scope of Work and Technical Specifications dated 6/20/2023 has been updated and changes are reflected in Exhibit D Scope of Work and Technical Specifications Rev.1, dated 7/26/2023.

The changes are replacing the word “clinical” with “clinic”  in the following Sections:

• Section 1.0 Background, Item 1.1 Scope
• Section 2.0 Work Included/Purpose, Items: 2.1.1, 2.1.3, 2.4.2.1, 2.4.5.4, 2.4.5.7
• No other changes have been made to the Solicitation.

Addendum #3 is issued to add changes to N3B Subcontract of Agreement From 9100, ARTICLE 4 – START AND COMPLETION OF THE WORK .
Replace N3B Subcontract of Agreement Form 9100, Rev. 3 (6/2023) with the attached N3B Subcontract of Agreement Form 9100, Rev. 4 (8/10/2023).

This Subcontract consists of three (1) base year periods, and includes three option(s) periods of continued performance/additional Work.

The Work shall commence as follows:
